Nella più ampia accezione, le terapie di supporto comprendono la profilassi e il trattamento sia degli effetti collaterali della terapia antineoplastica, sia dei sintomi della malattia.
Le terapie di supporto hanno ricevuto, finora, una relativamente scarsa attenzione da parte degli oncologi. Due esempi: solo il 50% dei pazienti con dolore neoplastico ha un soddisfacente controllo di tale sintomo, mentre si calcola che tale percentuale potrebbe salire al 90% se fossero adottate le più efficaci terapie antalgiche; la profilassi dell'emesi ritardata è raramente eseguita, malgrado le raccomandazioni contenute nelle linee guida formulate dalle più importanti associazioni scientifiche in campo oncologico. Tale situazione, però, è destinata a cambiare sotto una duplice spinta: la progressiva affermazione della centralità del ruolo del paziente nel processo decisionale che riguarda la sua cura e l'affacciarsi di nuove tossicità, dovute alle targeted therapies (ad es., tossicità cutanea, cardiotossicità), che vanno a sommarsi a quelle indotte dalla chemioterapia tradizionale.
Se da un lato si avverte sempre più il bisogno di una rigorosa ricerca clinica volta all'individuazione delle terapie di supporto ottimali, dall'altro è necessario che i risultati degli studi clinici in tale settore vengano rapidamente portati a conoscenza dell'oncologo. Inoltre, a guardare la crescente voluminosità delle riviste specialistiche, non si può certo dire che non si faccia ricerca su questi argomenti. Purtroppo, però, spesso gli studi riguardano aspetti marginali e comunque quasi mai sono rivolti all'individuazione di terapie ottimali per contrastare i singoli sintomi o loro cluster.
Da tali considerazioni emerge il ruolo di CASCO che ha per obiettivo principale la rapida diffusione delle informazioni sulle novità terapeutiche e la sensibilizzazione degli oncologi nei confronti delle terapie di supporto, affinché dedichino maggiore attenzione a tali aspetti, che sono di importanza tutt'altro che secondaria per il paziente.
Molte riviste si occupano di terapie di supporto e moltissimi articoli vengono pubblicati. CASCO assume anche la funzione di filtro, facendo selezionare da esperti gli studi che hanno maggiore rilevanza per il loro rigore e per la portata innovativa dei loro risultati. Da ultimo (the last, but not the least), la Rivista ha anche finalità formative analizzando la metodologia degli studi clinici in tema di terapie di supporto e discutendone i risultati, così da far acquisire al lettore competenze tali da consentirgli un'autonoma valutazione critica dei risultati degli studi pub
